An individual plan and no unproven methods: in Ukraine, the licensing conditions for the provision of rehabilitation services have been changed

On November 29, changes to the licensing conditions for rehabilitation institutions, departments and specialists that provide rehabilitation services came into force.
As Vasyl Strilka, director of the Department of High-tech Medical Care and Innovations of the Ministry of Health of Ukraine, told “UP.Zhyttia”, the new licensing requirements are aimed at ensuring the quality of rehabilitation.
“Now, the Cabinet of Ministers decree states that rehabilitation assistance should begin with a rehabilitation examination, during which it will be determined what kind of assistance a particular person needs and what result he wants to achieve.”he says.
According to Vasyl Strelka, after the examination, the patient’s individual rehabilitation plan must be developed and approved. This document is developed by rehabilitation specialists with the involvement of the patient and taking into account his needs and wishes.

“For example, a person after a stroke with impaired functioning of the hands is asked: “What do you most want?” She answers that to hug her grandchild. This is important to know, because rehabilitation is impossible without the “inclusion” of a person. Rehabilitation in the field of health care is aimed at for a person to achieve his needs and desires”– explains the specialist of the Ministry of Health.
A multidisciplinary team will work on rehabilitation. Tasks will be prescribed for each specialist in the individual rehabilitation plan.
“For example, the head of the team is a doctor of physical rehabilitation medicine, but a physical therapist will also work with the patient – after the restoration of motor functions, his task will be to teach the person to live with the limitations that may remain, as well as to take care of himself.
In order to motivate a person to actively participate in the rehabilitation process, a psychologist or psychotherapist will work. If the patient is an amputee, the multidisciplinary team will also include a prosthetist.”says Vasyl Strelka.

Mandatory individual rehabilitation plan is fixed in license conditions. The Ministry of Health says that the state is ready to pay for rehabilitation assistance, but wants to know what exactly the funds are being used for and to control costs.
“Because there are different cases: there are queues in some rehabilitation institutions and departments that provide good services, while others do not work as well as they are capable of. However, the state pays the same for both of them. Licensing conditions will help control the provision of rehabilitation services to patients and apply sanctions for their non-fulfilment”. – explains the specialist of the ministry.
If violations are detected during the monitoring of the institution, it can take away its license to provide rehabilitation services and deprive the National Health Service of Ukraine of contracting.

The new licensing conditions also aim to reduce the likelihood of using unproven rehabilitation methods.
“This problem has a long history. Since Soviet times, people are used to the fact that when they go to a sanatorium, they are smeared with mud, paraffin therapy and other unhygienic procedures are performed there. There is still a demand for them, and therefore there are those who provide such services. However, they do not have a proven effect, so the state will not pay for this type of rehabilitation,” – emphasizes Vasyl Strelka.

In addition, from now on, the licensing of rehabilitation facilities that are not health care facilities is being introduced – that is, they are not staffed by doctors, but by physical therapists.
“This will contribute to bringing out of the shadows rehabilitation specialists who provide services outside medical institutions in the so-called monoprofessional environment,” – the Ministry of Health claims.
Vasyl Strilka explains: since the 1990s, physical culture institutes trained rehabilitation specialists who now work as physical therapists, but for many years they could not legally provide separate rehabilitation services.
“Before the adoption of the law on rehabilitation in the field of health care, there was no state infrastructure for recovery, for example, after a stroke, and trained specialists could not open a private practice due to the lack of licensing. There is no other way out, except to provide such home care at home for direct payment from the introduction of the possibility of licensing the providers of such services should bring them “out of the shadows”, – says the specialist.
