Rehabilitation can be active (where the patient does exercises to restore the function of the limb) and passive (for example, electrophoresis or massage).

Why passive rehabilitation is called ineffective, explained Kateryna Zinovyeva, a doctor of physical rehabilitation medicine, in a comment for “UP.Zhyttia”.

Physical therapy and occupational therapy are active evidence-based rehabilitation. It involves working with the patient: restoration of movement, self-care skills, functioning of limbs or certain muscles. That is, a whole complex that will allow the patient to maximally adapt and adapt to life after the injury“, the specialist notes.

Electrophoresis, ultrasound, magnetic therapy, massages, Darsonval are examples of passive rehabilitation. It can be used as an additional method.

For example, paraffin therapy can be used to relax spasticity (increased muscle tone – ed.), and functional electrical stimulation is used to relax or, conversely, increase muscle strength.

For fractures, lymphatic drainage massage can be prescribed to reduce swelling. But then there should be a stage of active rehabilitation, where the patient, together with the occupational therapist, will work on the development of contractures (limitation of joint mobility – ed.), flexibility, muscle strength, etc.

“Very often in those physiotherapy offices that remain, there is a change of concepts: hardware therapy is called rehabilitation, although in fact it is not. For example, patients with back pain are prescribed a course of electrophoresis or magnetic therapy instead of testing the muscles – which are weaker, which are spasmodic“, explains the doctor of physical and rehabilitation medicine.

Kateryna Zinovieva adds that she has met departments of physical therapy abroad in many rehabilitation centers. But this method is prescribed no more often than once every 1-2 weeks.

There are patients who find it difficult to constantly work physically, they need a rest somewhere. So that they do not think that they have come and are doing nothing – they are sent to hardware physiotherapy for psychological support. If the person feels better and is ready to go and exercise, then the placebo effect of the hardware therapy has worked in our favor.”– says the specialist.

The doctor notes that in her practice there were cases when patients said – they told us to come to you for rehabilitation, we need magnetic therapy. And when these people heard that there is no such thing here – there are only sets of exercises, they turned around and left.

A magnet cannot restore muscle strength or balance the strength of the right and left sides, change people’s daily habits. This is possible only through active rehabilitation with a team of specialists.

There was a patient who said: if there are no devices, it means rehabilitation too, and then she concluded that we will not help her“, Kateryna Zinovieva recalls with a laugh.

She notes that active rehabilitation is individual work, rather than a general outline.

Specialists work on the patient’s goal, based on his capabilities. If the patient is lying in bed after a stroke, brain injury, or after a broken leg, he must first be taught to turn over in bed, sit down, and retrieve objects from the bedside table – that is, to teach the basics of self-care. This knowledge is not acquired in classes in physical therapy“, the specialist explains.

Kateryna Zinovieva advises not to be shy and ask a rehabilitation specialist about his education, because a bachelor of any specialty can become a physical therapist.

Before starting active rehabilitation, the specialist should pay attention to the patient’s condition – pressure, temperature, the presence of an inflammatory process or active healing of wounds or fractures. This is necessary for maximum comfort and benefit of the recovery process.

We would like to remind you that as of December 1, Ukraine changed the licensing conditions for the provision of rehabilitation services. From now on, only the individual plan will work and there will be no unproven methods.
